Further to our recent newsletter,
the Government has today released a final definitive version of the
licensing application forms to be used during transition. These are
slightly different to the ones released earlier and published on our
website for members to use and they have asked us to ensure that only
the new forms are used. These are available from our website or by
clicking the links below.
In addition, DCMS has now released the forms of consent to accompany
the forms - consent of the individual to act as designated premises
supervisor and the consent of existing licensees to the company holding
the premises licence - as well as the declaration form to accompany
personal licence applications stating criminal convictions. These
forms are available from DCMS
These forms are only to
be used during transition to convert existing licences and we will
shortly make available forms for new applicants to use.
The application forms have now been published and you can download
your copies from our website. The Form A and B is used to convert
and vary a premises
licence, and the individual Form is used for personal
licence applications.
